OFC tryout forms
All forms will be provided at tryouts but if you would like to have it all ready before that day, this is what you will need to bring:
1. USYA form-available to print (under the forms tab)
2. Medical Release-available to print-needs to be notarized (under the forms tab)
3. Athlete’s Code of Conduct-available to print (under the forms tab)
4. 2 copies of the players birth certificate
5. 2 small pictures of the player’s face
6. Registration check for 235.00-boys made out to ESC, girls made out to NOKC
a. The option to write two checks for registration is available, $135.00 due day of tryouts and one post dated 11/1/10.
